Founder, Chief Medical Officer
Chuback Vein Center was founded by Dr. Chuback in 2005 and has grown into a 5-Star, premier center for the contemporary treatment of vascular disease, varicose veins, spider veins, wound care, and lymphedema.
Along with the vein center, he established a state-of-the-art vascular imaging center on-premises, Chuback Vascular Imaging. Both Chuback Vein Center and Chuback Vascular Imaging are accredited by the Intersocietal Accreditation Commission, IAC.
Dr. Chuback is board certified in General Surgery, Cardiothoracic and Vascular Surgery and is a Diplomat of the American Board of Vein and Lymphatic Surgery. He specializes in minimally-invasive, same-day, laser treatment of veins performed in a comfortable office setting. Dr. Chuback firmly upholds the sanctity of the Doctor-Patient relationship by getting to know each of his patients, listening to their concerns, and crafting a tailored, individualized treatment for each one.
In 2010, Chuback Vein Center became part of Chuback Vein Center in Paramus, NJ. His team at Chuback Vein Center are board-certified, 5-Star reviewed medical professionals.
Educational Background: Dr. Chuback is a native of Bergen County and graduated from the Dwight Englewood School. He earned his M.D. degree from Rutgers Medical School in Newark, NJ. Dr. Chuback then completed a five year General Surgical Residency at Monmouth Medical Center. Besides an outstanding clinical and academic experience at the main institution, the program included intensive surgical training at Beth Israel Hospital in Newark, Memorial Sloan Kettering Cancer Center in Manhattan, Hahnemann University Hospital in Philadelphia, and Crozier-Chester Medical Burn Center.
Subsequently, he advanced his training further, undertaking a two-year advanced fellowship in Cardiovascular and Thoracic Surgery at The University of Rochester Medical Center and the internationally renowned Strong Memorial Hospital in New York. The Doctor is on staff at Valley Hospital in Ridgewood, NJ, and Chilton Memorial Hospital in Pompton Plains, NJ. Dr. Chuback is a Fellow of The American College of Surgeons, The International College of Surgeons, The American College of Thoracic Surgery, a member of the Society for Vascular Surgery, the Society of Thoracic Surgeons, and many other professional associations.
In addition to his passion for medicine, Dr. Chuback is a published author, U.S. patent-holding inventor, and entrepreneur. He founded Chuback Education and has written two wonderful books on personal development and success called Kaboing! 50 Ideas That Will Springboard You to Academic Greatness and Make Your Own Damn Cheese: Understanding, Navigating, and Mastering the 3 Mazes of Success. Dr. Chuback is also co-founder of VitaSupportMD, a nutraceutical company that produces high-quality dietary supplements that promote wellness and good health.
The doctor is married to Diane Chuback, Ph.D., MBA who serves as CEO of Chuback Vein Center. Together, they have 3 beautiful children of whom they are both very proud.

Since co-founding Chuback Vein Center in 2005 with her husband, Dr. John Chuback, Diane Chuback has helped guide the practice’s growth into one of the region’s premier medical centers dedicated exclusively to the diagnosis and treatment of chronic venous insufficiency and venous disease.
As Chief Executive Officer, Diane has focused on building the administrative and human resources foundation of the practice while working closely with the leadership team to support its continued growth. Together, they have assembled an exceptional team of professionals committed to delivering outstanding patient care and an experience built on compassion, education, and trust.
Diane believes that great patient care begins with investing in people. She is passionate about creating a workplace where team members are encouraged to continue learning, develop professionally, and find purpose in the work they do.

Education has been a constant throughout Diane’s career. She earned her undergraduate degree in Psychobiology from New York University before completing her Ph.D. in Neuroscience at Rutgers University. She later taught Biology and Neuroscience at Drew University, William Paterson University, and the State University of New York at Brockport. After several years in healthcare leadership, she returned to graduate school to earn an MBA from Ramapo College, strengthening the business foundation needed to help lead a growing medical practice.
A firm believer that education should extend beyond the walls of the office, Diane has long been committed to helping patients better understand their health and treatment options. She and her husband, Dr. John Chuback, co-founded LymphCast, a podcast created to increase awareness of venous and lymphatic disease through conversations with physicians, researchers, and innovators dedicated to advancing patient care.
Diane’s lifelong commitment to education continues to shape her work today. Her own journey through motherhood, combined with decades of leadership in healthcare, has renewed her interest in Neuroscience - not simply as a field of study, but as a way of understanding how people change and grow. Through her writing, she explores how the neurobiology of motherhood can illuminate broader lessons about leadership, resilience, and the extraordinary capacity for human transformation.

Chief Operations Officer
Maria Hughes is the Chief Operating Officer at Chuback Vein Center, where she has been a key part of the team for over 20 years.
With a background in business operations, healthcare management, and patient care coordination, Maria manages the practice’s daily functions while contributing to growth, efficiency, and providing an excellent patient experience.
Known for her “all hands on deck” approach, Maria excels at building strong teams, creating systems that support staff and patients, and helping bring Dr. Chuback’s vision to life. What she loves most about her role is being part of a team that genuinely changes lives and treats patients like family.
Beyond her leadership at the practice, Maria also plays a role in the growth and operations of VitasupportMD, where she handles branding, operations, and business development.
Outside of work, Maria enjoys spending time with family and friends, traveling, decorating, fitness and wellness, and encouraging those around her to succeed both personally and professionally. Her life philosophy is simple: work hard, stay loyal, be kind, and always lead with heart.

Director, Vascular Lab
Raised in Paramus, New Jersey, Mazin Radwan is the Vascular Lab Director at Chuback Vein Center.
He earned his credentials through the Cardiovascular Credentialing International (CCI) and is certified as a Registered Vascular Specialist (RVS) and a Registered Phlebology Sonographer (RPhS). His professional affiliates include the Society of Vascular Ultrasound. With almost a decade of experience in the field as a Registered Technologist, he has been with Chuback Vein Center since 2017. Mazin specializes in the performance of diagnostic imaging exams with the use of sophisticated noninvasive diagnostic imaging equipment. Mazin's goals in his career have always been to provide exceptional patient care and make an impact on people’s lives in a positive way.
A graduate of Eastwick College's Vascular Ultrasound program, Mazin graduated in the top 5% of his class. After his education, he began clinical work at one of the best hospitals in the country, New York-Presbyterian Columbia Medical University, with rotations in the ER, ICU, Neonatal ICU, pediatrics, transplant surgery, and outpatient care. His experience has expanded his skillset into multiple areas of vascular ultrasound, including but not limited to, cerebrovascular, renal vascular, peripheral arterial disease, and venous insufficiency. In his almost 10 year career he has assisted and guided surgeons with the use of ultrasound in over 700 endovenous laser therapy (EVLT) procedures, sclerotherapy (UGS), angioplasty, atherectomy, and arteriovenous fistula creation surgeries. After several years in hospital-based care, Mazin decided to move to private practice facilities with a strong focus on the diagnosis and treatment of Venous Disease. His move to private practice was underscored by the ability to provide more specialized premier patient care with an unparalleled patient experience. Finding a place that encompassed all of these values is what made Chuback Vein Center the perfect fit.
Surgical Coordinator
Christina, who many call Tina, is a passionate individual when it comes to helping others.
With over six years of experience in patient coordination and administration for clinics throughout Bergen and Passaic Counties, Christina is often the first welcoming face our patients see when entering Chuback Vein Center. She kindly greets people walking into our practice and can expertly guide those looking to inquire about Chronic Venous Insufficiency through their insurance verification process. She loves making people feel special and keeping patient satisfaction high ensuring that our patients are best taken care of from their first step into our office.
Christina earned her Bachelor of Science in Public Health at William Paterson University and continued professional education at Rutgers University in Environmental Health. Throughout her academic years as a Public Health undergrad, she has been committed to understanding and critically analyzing public health issues and helping improve the health of her community. Outside of work, you can catch Christina dancing, shopping or going to the movies. She loves photography and capturing moments of her friends and family. She is always looking to grow and learn new skills and loves being a part of Team Chuback, where she can do just that!
Surgical Coordinator
She has a background with over 10 years of experience in the medical field.
As a surgical coordinator, she enjoys serving patients and trying to make each appointment as easy, pleasant, and memorable as possible. She enjoys the relationships formed with our patients and watching the state of their health and wellbeing transform with every appointment! Cassandra’s warm smile and positive persona make all the difference in our patients’ experience.
When Cassandra isn’t at work, she enjoys spending her free time delving into her other interests. Some of them include teaching herself piano, reading books, outdoor activities, health/fitness, and spending time with her son. Her little family is a big part of her life and she considers herself lucky to have a second family here at Chuback Vein Center.
Lead Vascular Ultrasound Technologist
Mariely Fermin was born and raised in the Dominican Republic before immigrating to Paterson, New Jersey, at the age of 14.
Since entering the cardiovascular field in 2016, she has specialized in providing physicians and patients with high-quality diagnostic imaging through non-invasive ultrasound techniques. Mariely is an active member of the Society for Vascular Ultrasound and is deeply committed to delivering exceptional patient care while supporting patients throughout their treatment journey.
A graduate of Eastwick College, where she earned a Bachelor of Science in Diagnostic Cardiovascular Sonography, Mariely began her clinical career at Englewood Hospital and Medical Center, where she gained valuable outpatient care experience. Over the past nine years, she has developed extensive expertise in multiple areas of vascular ultrasound, including carotid artery imaging, upper extremity venous and arterial studies, peripheral arterial disease evaluation, and venous insufficiency assessment.
During her four years at Chuback Vein Center, Mariely has further expanded her diagnostic expertise while continuing to provide outstanding patient care. She has embraced new challenges, refined her technical and clinical skills, and taken pride in fostering a positive, collaborative work environment. Her dedication to continuous professional growth reflects her commitment to delivering the highest standard of care to every patient.
Outside of work, Mariely enjoys spending quality time with her family and watching films together. She lives by a simple philosophy: “Lead with kindness, work with purpose, and never stop learning.
Integrated Marketing Manager
Andrea Cabrera started her journey with the company as a Marketing Intern and soon after became a Social Media Assistant before landing her current position.
She holds a Bachelor's degree in Journalism, which she obtained in her home country of Ecuador in 2014. In 2018, she moved to the US to further her education and she graduated with an M.A in Professional Communications from the William Paterson University of New Jersey.
Andrea brings a unique blend of storytelling and marketing expertise to her role. For Andrea, the most rewarding aspect of her job is helping people find the necessary resources and support to improve their health and transform their lives positively.
Medical Assistant
Davianna “Davi” Ortiz is a dedicated Medical Assistant who has been working in the healthcare field since 2022.
Born and raised in The Bronx, Davi recently made the move to New Jersey nearly two years ago and has enjoyed adapting to the New Jersey lifestyle while continuing to grow professionally in healthcare.
Before joining Chuback Vein Center, Davi gained valuable experience working with NewYork-Presbyterian, where she strengthened her passion for patient care and teamwork. She takes pride in creating a welcoming and supportive environment for patients while providing compassionate, high-quality care.
Outside of work, Davi enjoys spending quality time with her family, friends, and partner, exploring new activities and making meaningful memories together.
